Desired over have
How much of a stock preparation to give: (desired ÷ have) × quantity. Quantity is the volume or tablet count that contains the “have” strength.
Amount to give
(desired ÷ have) × quantity
IV pump rate (mL/hr)
Infusion volume divided by time.
Rate
volume ÷ time
Manual drip rate (gtt/min)
Gravity tubing: (volume × drop factor) ÷ time, rounded to whole drops.
Drip rate

Total daily dose by weight, split into equal doses.

When: The workhorse formula for tablets and liquids.

mL/hr = volume (mL) ÷ time (hr)
The rate to program on an infusion pump.
When: Continuous infusions ordered over a number of hours.

gtt/min = (volume × drop factor) ÷ time (min)
Drops per minute for gravity tubing; round to whole drops.
When: Gravity infusions where you count the drip chamber.

Weight-based dose
dose = dose-per-kg × weight (kg)
Total dose scaled to the patient’s weight.
When: Pediatric and weight-based medication orders.

per dose = (dose-per-kg/day × weight) ÷ doses/day
Each administration when a daily dose is split evenly.
When: Orders written per day but given several times daily.
