Restraints are devices or methods that limit a client’s movement, used only as a last resort to protect the client or others when less-restrictive alternatives have failed. They require a provider order specifying the least-restrictive type and a time limit, frequent monitoring of circulation, skin, and toileting needs, and a quick-release knot tied to the bed frame — never to a side rail. Always try and document alternatives first, and remember orders are time-limited and must be renewed per policy rather than standing indefinitely.
